Telephone or video care and needs’ assessments
Before any Resident is admitted into a care home, we will carry out a full assessment of their care and personal needs so we can be sure that our care home is suitable for them and that they can be safe and happy.
Since the start of the pandemic we have carried out assessments by telephone or video call to minimise the risk to everyone of contracting the virus.
We can arrange a telephone or video assessment at a time to suit the wider family as it is helpful for Relatives to be involved as much as possible throughout the process of choosing the right care home for their loved one.
Personalised care planning
Prior to a Resident’s admission into the care home, our professional team will devise an initial care plan which will direct the way that appropriate care and services are provided that meet the Resident’s care needs and personal preferences. The care plan addresses the findings from the care and needs assessment so that they can move into the care home in the knowledge that their needs will be met.
Moving into our care home
Our professional team will work with you to agree the best possible day to move in, and we’ll help you every step of the way. We’ll provide you with guidance on all the things that you can do to make the transition as seamless as possible.
