Supporting discharge to assess this winter
As we head into a winter that is likely to see greater pressure on our health and social care systems than ever seen before, we stand with our NHS and local authority partners to provide hospital discharge services and high-quality care in our homes across the country.
We are dedicated to delivering D2A services (pathways 2 and 3) in an enabling and supportive manner, in addition to providing short-term stays for pathway 1 patients. HC-One is proud to offer specialist care and expertise in this area, ensuring assessments of need and eligibility take place in a caring environment with a planned and professional approach.
