At HC-One, it is our mission to be the first choice care home provider for older people and health and social care professionals in each of the communities we serve.

With more than 275 care homes, we are the largest provider of residential, dementia and nursing care in the UK. We work closely with health and social care professionals across over 100 Local Authorities and NHS Trusts to ensure we are delivering the kindest possible care to older people on both a permanent and short-term basis.

Our wealth of knowledge and sector expertise means we are proud to offer the very highest standards of residential, dementia and nursing care in a warm and welcoming environment, whether it be a short care break, a period of recovery after treatment in hospital, or a longer-term place to call home.

With care homes across England, Scotland and Wales, our national scale means we can mobilise rapidly and work in partnership with provider groups, such as Care England, Scottish Care and  Care Forum Wales, and central and devolved Governments, to help achieve fair settlement for providers and commissioners. We also have a dedicated procurement function to ensure we secure the best value contracts from our suppliers. However, our national scale is complimented by a personalised local approach. Our leadership and regional teams benefit from a wealth of sector expertise which enables us to work collaboratively with local authority and NHS partners to understand their individual needs to develop a tailored service.

A warm, welcoming home this winter

This winter, we are here to help ease pressure on frontline health and social care services by providing high quality care to those who need it most. We are proud to stand with our NHS and local authority partners by offering a full care pathway across our network of homes, to accommodate our Residents’ individual needs.

We will continue to deliver Discharge to Assess (D2A) services (pathways 2 and 3) in an enabling and supportive manner this winter, and beyond. In addition, we provide short-term stays for pathway 1 patients, as well as ongoing personalised care plans for those who require longer term solutions, including planned rehabilitation and reablement.

HC-One prides itself on having extensive experience in this particular area, having set up and delivered both block contracted and spot placement services over the last five years.

Our kind care commitment to our environment, our Colleagues and our communities

As the Kind Care Company, we are committed to being kind to our environment, our Colleagues, and our Communities, to ensure the best value is provided for our Residents. This includes a £54.5million refurbishment programme to improve the living environment in over 200 of our care homes, and a £17m investment in our pay and reward programme for Colleagues.

We are committed to supporting our Colleagues’ wellbeing, with a wealth of support available at HC-One, in addition to continuously investing in our team’s professional development and career progression with a transparent and fair approach to pay and reward. Our Colleagues receive training to ensure the short stay enablement principles are embedded in everything we do, enabling us to work in partnership with professionals and Colleagues to support the personalised outcomes needed by the people we look after.

What our commissioners have to say

We are proud to work closely with local authorities in each of our communities across the UK. Here is just some of the feedback we have received about the kind care we provide at HC-One:

“We have a long-standing relationship with HC-One for people requiring care services from both in the community and from hospitals. The support from HC-One’s services in East Sussex has helped to ease the pressures on acute wards within East Sussex. Services have been responsive and performed well within the agreements made for the Discharge to Assess model.”

East Sussex County Council

“After working closely over a number of years with HC-One care homes, we have always found them to be flexible, professional, have a detailed understanding of the wider health system pressures, and always ready to help wherever they can.”
Tameside Metropolitan Borough Council

“I have worked with HC-One for many years now, in particular to address system pressures during the winter period, and have always appreciated our partnership and have found them responsive, flexible and cooperative.”

Stockport Council
