We stand ready at all times of the year to help ease pressure on frontline health and social care services by providing high quality care to those who need it most by offering a full care pathway across our network of homes to accommodate our residents’ individual needs.
We provide intermediate care (also known as hospital discharge services, Discharge to Assess (D2A), or step-down care) in a collaborative and enabling way. We develop personalised care plans that can include planned rehabilitation and reablement to maximise independence and the chance of returning home. For individuals requiring longer-term care, we support transition into the next stage of their care journey. Our delivery is increasingly supported in HC-One homes through the use of digital care management tools, including Nourish and Radar.
We pride ourselves on having extensive experience in delivering successful hospital discharge pathways, having developed and delivered numerous block contracted and spot placement services over the last five years.

Supporting families with short term assessment stays
Our highly skilled care teams provide high quality 24-hour nursing care and support when somebody no longer needs to stay in hospital, but is not quite ready to return home.
We are dedicated to providing compassionate care and support right from the initial assessment, working with residents and their families to create a longer-term care and support plan, and working with professionals from the community including social workers and GPs to get the assistance that each person needs.
Enhanced Discharge to Assess
At HC-One, we are dedicated to supporting our communities, which includes working with our partners across health and social care to support patients and their families in their journey to recovery after time in hospital.
That’s why we have developed the Enhanced D2A Service, providing an efficient D2A pathway to help free up hospital beds, while ensuring patients who are discharged from an acute setting can rest and recover in a homely, caring environment.
Compared to longer-term care in one of our care homes, the aim of Enhanced D2A care is to support people in regaining their independent living skills, with the aim of leaving and returning home.
Our person-led approach to reablement means each patient using this service will be supported to build confidence and regain the skills they need to live independently.
The living environment of our Intermediate Care Centres is carefully designed to enable these outcomes, with access to facilities such as an enablement kitchen, and specialist exercise and rehab equipment, available.
